Lumbar puncture (LP) and Intrathecal Chemotherapy Administration

Overview

Sometimes, doctors need to check the fluid that surrounds your brain and spine to help with diagnosis, treatment, or to give medicine that helps protect the brain. This is done with a simple test called a lumbar puncture, also known as a spinal tap.

While the name might sound a little scary, it’s actually a safe, quick, and gentle procedure, and we make sure you’re as comfortable as possible every step of the way.

What to Expect

Pre-Procedure Instructions

Before the procedure, follow the fasting guidelines provided by your healthcare team and ensure your child understands the process in simple, reassuring terms to ease anxiety. You’ll be asked to sign an informed consent form detailing the procedure’s risks and benefits, while also disclosing any medications, allergies, or history of bleeding disorders

Procedure

The doctor gently inserts a tiny needle into the lower part of your back (well below the spinal cord). A small amount of spinal fluid is collected for testing, or medicine is given through the needle if needed.

It is done under sedation to ensure a comfortable experience for our patients.

Duration

The procedure typically takes about 30-45 minutes, patients will be positioned lying on one side with knees drawn to widen the spaces between vertebrae.

Post-Procedure Care and Complications

 

Intrathecal (IT) Chemotherapy Administration

We commonly use fluoroscopic guidance for intrathecal chemotherapy administration, which is performed by an experienced interventional radiologist. This considerably improves administration accuracy by giving real-time imaging throughout the process, allowing improved visualization of the anatomy of the spine to ensure exact needle placement in the subarachnoid space, reducing the chance of problems such as medication misplacement. This is extremely significant in leukemia patients, because traumatic tap can modify the disease status and treatment approach.
